Applicants may be required to provide:

• A valid Ghana Card or other approved national identification
• Evidence of active NHIS membership
• A medical referral letter
• A diagnosis or medical report from a certified practitioner
• Relevant laboratory, imaging or pathology results
• A proposed treatment plan
• A Multidisciplinary Team recommendation, where required
• A completed GMTF application form

Additional documents may be requested depending on the condition and proposed treatment.
